To return from a regular leave of absence, please submit this form at least six weeks prior to the start of the semester.

Reinstatement Form

An intent to complete form must be submitted according to the following deadlines:

• Fall semester completion:November 1
• Spring semester completion: March 1
• Summer semester completion: July 1

Successful completion of the program requires earning at least 30 units of approved course work (24 units of which must be completed at WUSTL) with a cumulative grade point average of 3.2 or higher. Up to six of the 30 units may be applied in transfer. Students who meet the completion criteria are eligible for a cover letter. Please see our program policies for more information.
